Dean PEKAROVIC, radiographer

sl2

Dean Pekarovic is Head Radiographer in Radiation Protection Office in the University Clinical Centre in Ljubljana, Slovenia.

He worked as head radiographer in advanced modality (CT/MR) department for 10 years where he started with implementation of quality system. In last year he developed a comprehensive training program on technical quality for radiology residents and radiographers. He established and is now in charge of Center for quality and safety of radiological equipment with a group of specially trained radiographers dosymetrist who are responsible for technical quality of all equipment in the University Clinical Centre. His research in last years is targeted to optimisation of radiological practices, especially in digital modalities. He is invited speaker in many workshops and national congresses talking about the importance of targeted training for radiographers in radiation protection.

Major Reference

He was active as initiative board EFRS, lately as treasurer and advisory board members in EFRS. At the moment he lead CT expert group in EFRS. In last 5 years he was invited as invited speaker in many international events (Subcommittee chairman in ECR 2013- Radiographers programme) and workshops. He actively participated as EFRS representative in different EU project regarding Radiation Protection such as: EMAN – European Medical Alara Network, MEDRAPET (Medical Radiation Protection Education & Training), PiDRL (European Diagnostic Reference Levels for Paediatric Imaging), ESR EUROSAFE CT expert radiographer.

Practice Activities

Currently, 60-70% of my time is spent in clinical practice activities in chest and cardiac imaging section.

List all clinical activities, both those at Harvard and its affiliates and those outside Harvard, and for each indicate:

Year(s)

Type of activity

Name and location of practice

Level of activity

Radiology
Cardiac CT, MR and Thoracic imaging (CT, plain radiography and MRI)
Take part in and developed CT scanning protocols, techniques and radiation dose techniques in pediatric radiology, abdominal and chest radiology

Massachusetts General Hospital (teaching hospital)
Boston, Massachusetts

All levels of complexities of cases are seen at MGH radiology’s cardiac imaging and thoracic radiology sections. The load of patient for MGH radiology is heavy to very heavy.

Clinical Innovations

Optimization of automatic exposure control technique for adult CT radiation dose reduction. 2004-05

Helped reduce radiation by 25-66% in adults for chest, abdominal and renal stone protocols in the department. This has been reported and published in several meetings and international peer review journals. This system of dose optimization has been applied in many institutions since then including MGH, NSMC, NWH, Dana Faulkner and Emory University Hospital.

Optimization of iterative reconstruction protocols based for chest and abdominal CT dose reduction 2008-10

Helped reduce radiation dose by 25-40% compared to the conventional technique for adult CT protocols in chest and abdomen. These findings have been reported in the Radiology and Journal of Computed Axial Tomography in 2009 and 2010. I was invited to talk on this at the GE Healthcare, Waukesha, Wis.

Pedizone CT Dose optimization protocols for children 2007-08

Helped reduce pediatric CT radiation dose by about 80% in the MGH. In addition, these protocols have been published and actively circulated by major CT equipment vendor around the world. Presented at Radiological Society of North America in 2008

Pedizone CT Dose optimization protocols for children with iterative reconstruction technique 2009-10

Helped reduce pediatric CT radiation dose by about 30-50% in the MGH over and above the initial protocols in the preceding section. In addition, these protocols have been published and actively circulated by major CT equipment vendor around the world. Presented at Radiological Society of North America in 2009

Indication based and BMI based cardiac CT protocols. 2008-2009

Have helped reduce radiation dose for cardiac CT protocols based on multiple factors such as patients’ heart rate, coronary calcium score, age, as well as body mass index.

Development of Dual energy CT protocols for pulmonary embolism

Enhanced patient care, and improved the diagnostic quality and information from these crucial protocols. In comparison with Single energy CT, we were able to even reduce radiation dose with DECT protocols for this specific indication.

Virtual autopsy services

Founded and led the virtual autopsy capabilities in our department and hospital. Developed logistics, forged partnership with autopsy pathology section and ultimately developed both CT and MR imaging protocols, reporting template. Established rich and massive imaging and sonogram database de novo, which spans close to 50 TB and counting. Obtained an educational grant from the RSNA to develop this program and published several articles.

Training for low radiation dose CT for radiologists

Created and conducted a hands on interactive training program on educating radiologist on image interpretation at low radiation dose CT

Dual energy CT for routine chest protocol CT

Novel contrast injection protocols for non-DECT capable CT systems

CT protocols for evaluation of Hereditary hemorrhagic telengiectesia

Special low dose chest CT protocols for lung cancer screening, lung nodule follow up

Fast Chest CT protocols

Low contrast volume PE CT studies

Novel vascular CT protocols

Developed special contrast injection technique to enable simultaneous vascular and parenchymal enhancement. Submitted several studies on this technique. Reported that this technique can virtually enable similar information as pulmonary embolism protocol with less complexity.

Developed and validated use of new split bolus contrast injection protocols for older generation of the CT systems in order to accomplish simultaneous vascular and parenchymal enhancement in the thorax.

This was developed on special request from the HHT center in MGH where we use the latest CT equipment with specially created DECT protocol which I have developed. We basically can obtain much greater information in terms of detection of smaller AVM in the lungs while also getting information on lung perfusion at much lower radiation dose level compared to routine pulmonary embolism protocol.

These protocols are associated with lower radiation dose than the routine chest CT protocols and encompass 16 CT systems from three different CT vendors both with and without iterative reconstruction techniques

Several severely ill patients or incapacitated patients are unable to hold their breath or follow instruction for avoiding motion during CT scanning. I developed special non-breath-hold CT protocols, which are 2-3 times faster than the general routine chest CT protocol and helps decrease motion artifacts.

Patients with compromised renal functions can have CT with this protocol using a fraction of contrast needed otherwise. This contrast volume of about 25-35 ml can also be given with diagnostic studies following an initial non-diagnostic CTA

Application of dual energy CT to CTA of the body. Development of advanced 4D CT and novel injection protocols for scanning patients undergoing vascular CT.
